Health Requirements


*For our Pre-K students, only a copy of your child’s immunizations is required. All other state mandated medical forms begin when your child enters Kindergarten.*

The following is a list of health requirements required by the state of Pennsylvania and Our Lady of Good Counsel School for admission into Kindergarten. Some forms require updates when certain grades are reached. Please see below for more details on those forms as well as the grade level that require updated information:
1.  Copy of Up-to-Date Immunization Records
2.  Pennsylvania Physical Health Examination
3.  Health History
4.  Dental Examination
5. Medication Form
6. Asthma Action Plan
7. Allergy Action Plan
8. Seizure Action Plan

Copy of Up-to-Date Immunization Records
Students who fail to meet all immunization requirements will be conditionally enrolled and then excluded from school until the immunization requirements are met as stated by the Pennsylvania Department of Health.

Pennsylvania Physical Health Examination (Admission to Kindergarten & updated in Grade 6)
The Pennsylvania School Health law requires that students have a physical examination dated after January 1st of the current year on original entry to school (Kindergarten), grades six and eleven, either by the private physician or school doctor.  Students transferring to the Centennial School District without record of a recent examination in the previous school shall be required to have a physical examination. Written parent/guardian permission will be required for the physical examination to be performed in school. Click to view the: PA Physical Examination Form.

Health History
This Health Services History Form must be completed and is specific to Centennial School District, the district in which Our Lady of Good Counsel resides.

Dental Examination (Admission to Kindergarten & updated in Grades 3 and 7)
The Pennsylvania School Health law requires that students have a dental examination, dated after January 1st, prior to original entry to school (Kindergarten), grades three and seven, either by private dentist or school dentist. Students transferring to the Centennial School District without record of a recent dental examination in the previous school will be required to have a dental examination. Medication Form
If your child should for any reason need medication administered to them throughout the school day, please fill out this Medication Form and send it to the Nurse’s Office & School Office.

Asthma Action Plan
If your child has asthma, please fill out this Asthma Action Plan Form and send it to the Nurse’s Office & School Office.

Allergy Action Plan
If your child has allergies, please fill out this Allergy Action Plan Form and send it to the Nurse’s Office & School Office

Seizure Action Plan
If your child is prone to seizures, please fill out this Seizure Action Plan Form and send it to the Nurse’s Office & School Office.
