The following is a list of health requirements required by the state of Pennsylvania and Our Lady of Good Counsel School for admission into Kindergarten. Some forms require updates when certain grades are reached. Please see below for more details on those forms as well as the grade level that requires updated information:
Copy of Up-to-Date Immunization Records (Exemptions NOT accepted)
Students who fail to meet all immunization requirements will be conditionally enrolled and then excluded from school until the immunization requirements are met as stated by the Pennsylvania Department of Health. Exemptions are NOT accepted at OLGC School.
Pennsylvania Physical Health Examination (Admission to Kindergarten and updated in Grade 6)
The Pennsylvania School Health law requires that students have a physical examination dated after January 1st of the current year on original entry to school (Kindergarten), grades six and eleven, either by the private physician or school doctor. Students transferring to the Centennial School District without record of a recent examination in the previous school shall be required to have a physical examination. Written parent/guardian permission will be required for the physical examination to be performed in school. The PA Physical Examination Form can be found below.
Health Services History
The Health Services History Form must be completed and is specific to Centennial School District, the district in which Our Lady of Good Counsel resides. The Health Services History Form can be found below.
Dental Examination (Admission to Kindergarten and updated in Grades 3 and 7)
The Pennsylvania School Health law requires that students have a dental examination, dated after January 1st, prior to original entry to school (Kindergarten), grades three and seven, either by private dentist or school dentist. Students transferring to the Centennial School District without record of a recent dental examination in the previous school will be required to have a dental examination. Written parent/guardian permission will be required for the dental examination to be performed in school. The Dental Examination Form can be found below.
Medication Form
If your child should for any reason need medication administered to them throughout the school day, please fill out the Medication Form found below and send it to the Nurse’s Office.
Allergy Action Plan
If your child has allergies, please fill out the Allergy Action Plan Form found below and send it to the Nurse’s Office.
Asthma Action Plan
If your child has asthma, please fill out the Asthma Action Plan Form found below and send it to the Nurse’s Office.
Seizure Action Plan
If your child is prone to seizures, please fill out the Seizure Action Plan Form found below and send it to the Nurse’s Office.
